Отдаленные результаты различных методов реваскуляризации миокарда у больных ишемической болезнью сердца

Abstract

Percutaneous coronary interventions (PCI) with stenting and coronary artery bypass grafting (CABG) are increasingly used to treat complex coronary artery disease. Our study included patients (n=450) with multi-vessel or left main coronary artery disease underwent CABG or PCI. In long-term period (12, 36, 60 month after revascularization) were evaluated 104 patients. The primary clinical endpoints were estimated as major cardiovascular events, as cardiac death, stent thrombosis, in-stent restenosis, myocardial infarction. Secondary study endpoint were repeated cases of myocardial revascularization at 12, 36, 60-months period after surgery. Most of the preoperative characteristics were similar in two groups. Rates of major adverse cardiac events at 12, 36, 60 months were significantly higher in the PCI group, in large part because of an increased rate of repeat revascularization. CABG remains the standard care procedure for patients with multi-vessel coronary artery disease, since the use of CABG, as compared with PCI, resulted in lower rates of the combined end point of major adverse cardiac events and revascularization at 1, 3, 5 years.
