Интервальное гипоксическое кондиционирование: системные и молекулярные механизмы, возможности использования в клинической практике

Аннотация
Подход с использованием интервального гипоксического кондиционирования основан на чередовании воздействия гипоксических и нормо-/гипероксических условий, облегчая адаптацию к пониженной доступности кислорода. Эта методика, изначально используемая в спорте и высотной медицине, показала себя многообещающей при лечении и реабилитации пациентов со множеством заболеваний.
В данном обзоре систематизированы имеющиеся опубликованные данные относительно роли прерывистого гипоксического кондиционирования и его основных механизмов в качестве потенциала в защите органов и лечении широкого спектра заболеваний. Инициируемые гипоксическим кондиционированием адаптационные механизмы включают ряд процессов, таких как улучшение функции митохондрий, снижение окислительного стресса, воспаления и апоптоза, а также усиление экспрессии гипоксических генов, улучшение метаболического статуса.
Потенциальные положительные эффекты гипоксического кондиционирования были продемонстрированы во многих областях, показав клиническую эффективность: например, снижение артериального давления у больных с артериальной гипертензией, уменьшение симптомов коронарной недостаточности у больных со стабильной ишемической болезнью сердца, более быстрое и полное восстановление после операции аортокоронарного шунтирования, сохранение когнитивных функций у пожилых людей, содействие снижению массы тела у людей с избыточной массой тела.
С учетом того, что прерывистое гипоксическое кондиционирование способствует полезным физиологическим реакциям во многих органах и системах, это исследование направлено на привлечение внимания к возможности клинического применения интервального гипоксического кондиционирования в качестве многоорганной защитной стратегии.
