Аннотация

Синдром обструктивного апноэ сна (СОАС) является модифицируемым фактором риска сердечно- сосудистых заболеваний. Неупорядоченные дыхательные события вызывают прерывистую гипоксию, окислительный стресс, симпатическую активацию и эндотелиальную дисфункцию, все они являются критическими медиаторами сердечно-сосудистых событий. Лечение СОАС непрерывным положительным давлением в дыхательных путях (CPAP) успешно применяется для лечения ишемической болезни сердца, гипертонической болезни, нарушений ритма сердца, инсультов, сахарного диабета, легочной гипертензии, сердечной недостаточности. CPAP-терапия является длительным, иногда пожизненным методом лечения, большое значение имеет приверженность пациента к лечению. «Золотым стандартом» считается проведение CPAP-терапии каждую ночь в течение всего времени сна. При проведении краткосрочных исследований было показано, что увеличение продолжительности эффективной CPAP-терапии может привести к большему снижению сердечно-сосудистого риска. Ведется работа по определению основных стратегий улучшения приверженности к CPAP-терапии на длительное время и определения минимальных доз CPAP-терапии, необходимых для уменьшения сердечно-сосудистого риска. Эта необходимость особенно важна в связи с высоким риском сердечно- сосудистых осложнений среди населения. В настоящем обзоре проанализированы данные современных исследований, оценена эффективность CPAP-терапии у пациентов с сердечно-сосудистыми заболе- ваниями.
