Современные аспекты фармакогенетики клопидогрела и его клиническое значение

Аннотация

Основными причинами смертности от сердечно-сосудистых заболеваний являются атеротромботические осложнения. Наиболее частый патофизиологический процесс, лежащий в их основе, – формирование окклюзирующего тромба с большим содержанием тромбоцитов в месте разрыва атеросклеротической бляшки. Поэтому антитромбоцитарные препараты являются одним из основных направлений в лечении больных ишемической болезнью сердца и обеспечивают профилактику развития тромботических осложнений. Формирование оптимального ингибирования тромбоцитов зависит от степени ишемического риска, в противовес которому вступает риск развития кровотечений. Клопидогрел применяется с целью профилактики повторных ишемических событий после острого коронарного синдрома и/или чрескожного коронарного вмешательства. Сниженный ответ тромбоцитов на этот препарат (высокая остаточная реактивность тромбоцитов) был определен как фактор риска развития повторных ишемических событий. Во многих исследованиях показана взаимосвязь между наличием генетических полиморфизом и снижением количества активного метаболита клопидогрела, что, в свою очередь, приводит к недостаточному антиагрегантному эффекту и развитию неблагоприятных клинических событий. Тем не менее пока не ясно насколько выраженно влияние полиморфизма на вариабельность ответа на клопидогрел.

Существует несколько типов аллелей, способных модифицировать активность белков, уменьшать или увеличивать концентрации активных метаболитов и влиять на эффект лекарственного средства. Было выявлено, что аллельный вариант CYP2C19 с утратой функции (в особенности аллель *2) является основным генетическим медиатором ответа тромбоцитов на клопидогрел. Носители данного аллельного варианта имеют более высокую реактивность тромбоцитов на фоне терапии лопидогрелом и высокий риск неблагоприятных сердечно-сосудистых событий, включая тромбоз стента, инфаркт миокарда и смерть. Носительство аллеля *2 CYP2C19 в сочетании со всеми демографическими и клиническими предикторами может объяснить лишь 12% высокой реактивности тромбоцитов, поэтому необходимо проводить дальнейший поиск причин. В данном обзоре изучается вклад генетических полиморфизмов, ответственных за вариабельность ответа на прием клопидогрела, и его клиническое значение.
