Abstract

Myocardial bridging of the coronary artery (MMCA) is a congenital anomaly in which a portion of the CA passes under the epicardial surface of the myocardium, instead of its usual path along the epicardial surface. This deviation of normal arterial anatomy can have various consequences and requires attention and analysis in this clinical case. The main instrumental methods for diagnosing MMCA: coronary angiography, CT angiography, stress echocardiography. In the presented case, computed tomography (CT) coronary angiography was performed on a 128-slice computed tomograph. In the middle section of the right coronary artery, a myocardial muscular bridge was detected over a length of 16 mm; the artery was “immersed” in the epicardium by 3.5 mm. The artery in the thickness of the myocardium is stenotic up to 25–30%. In addition to the muscular bridge of the right coronary artery, a heterogeneous atherosclerotic plaque of the anterior interventricular branch without pronounced stenosis was discovered. Awareness of the myocardial bridge of the right coronary artery is an important component for effective diagnosis, treatment and prevention of complications associated with this pathology.
