Adipokine status and its association with myocardial remodeling in patients with mitral valve disease

Abstract

Objective. To evaluate the concentration of adipokines in the blood of a cohort of patients with acquired mitral valve disease and the relationship between adipokine status and structural and functional parameters of the heart.

Material and methods. A total of 51 patients with indications for surgical correction of mitral valve defect and hospitalized for planned surgical intervention were included in the study. All patients underwent multispiral computed tomography of the abdominal cavity and heart to assess the area of visceral and volume of epicardial adipose tissue, transthoracic echocardiography with assessment of structural and functional disorders of the heart. All patients underwent blood sampling with an assessment of the serum level of adipokines (leptin, resistin, adiponectin).

Results. It was revealed that an increase in the blood concentration of leptin by 1 ng / ml is associated with an increase, and therefore with deterioration of the global longitudinal strain of the left ventricle (B-coefficient 0.09, p = 0.03), with deterioration of the systolic function of the right ventricle, reflected in decrease in right ventricular ejection fraction by B-coefficient –0.15, p = 0.045). In addition to the level of leptin in the blood, an increase in the leptin / adiponectin ratio by 1 ng / μg associated with deterioration of tricuspid annular plane systolic excursion (B-coefficient –0,104, p=0,029). An increase in adiponectin concentration by 1 μg/ml is associated with an increase in the indexed volume of the left and right atria (B-coefficient 3.37, p=0.02 and B-coefficient = 2.64, p=0.008, respectively). In addition, an increase in adiponectin and leptin levels is associated with an increase in the blood concentration of galectin-3.

Conclusion. An increase in the level of adipokines in the blood is associated with structural and functional changes in the both left and right heart. An association of adipokines with an increase in the blood level of a marker of cardiac remodeling and myocardial fibrosis (galectin-3) was revealed.
