Abstract

Tetralogy of Fallot (TF) is the most common congenital heart defect (CHD) of the cyanotic type. Since the first radical correction was performed in 1954, the tactics of treatment of this CHD has been constantly improving. Currently used TF treatment strategies are associated with a high level of long-term survival (30-year survival ranges from 68.5% to 90.5%). However, after performing a radical correction of TF, the patient may have a number of problems: persistent obstruction of right ventricular (RV) outflow tract, pulmonary artery valve (PAv) regurgitation, ventricular arrhythmias. These pathological phenomena are widespread in this cohort of patients and often require repeated interventions. The often cause of RV dysfunction is a long-term regurgitation on the PAv, as well as its stenosis. Myocardial remodeling acts as the main cause of ventricular and supraventricular arrhythmias, which determines the need for both drug correction and the implementation of interventions aimed at preventing sudden cardiac death in these cohort of patients. The presented review discusses the main pathological conditions that limit the duration and quality of life in patients after radical correction of classical TF, also as possible ways to solve existing postoperative complications
