Abstract

Objective. To evaluate interobserver reproducibility and error of multislice computed tomography (MSCT) in the aspect of studying the anatomy of the pulmonary arteries (PA) in patients with congenital heart defects with univentricular hemodynamics.

Material and methods. The study included 25 patients with a functioning bidirectional cavopulmonary anastomosis and 3 phantoms. In the patient group, morphometry was performed with an assessment of the Nakata index, McGoon effective (the average value between the maximum and minimum McGoon index) using MSCT angiography; in the phantom group, the diameters of the models of the main branches of the PA at the level of the ostium were measured using a caliper and MSCT. Interobserver reproducibility in the patient group was compared between three radiologists, in the phantom group – between two radiologists.

Results. The PA morphometry in the group of patients performed by the first, second and third radiologists had the following average values: McGoon ratio effective – 2.5, 2.4, 2.4 (p > 0.05), Nakata index – 296, 282, 295 (p > 0.05). The agreement range (intraclass correlation coefficient, ICC) between the three researchers in assessing the PAI (pulmonary arterial indices) was within the limits of good and excellent reliability. The results of comparisons of the real sizes of the PA branches in phantoms with the measurements obtained by MSCT – the error of the method is 2.7%, the average measurement error of one researcher is 4.3%. No significant differences were found between repeated measurements performed by independent specialists in the phantom group (p > 0.05), significant discrepancies in values beyond two standard deviations were detected only in 6.6% of cases. The obtained values did not exceed the permissible level for assessing the reproducibility of medical research results equal to 10%.

Conclusion. Assessment of the reproducibility and error of the research method showed the absence of significant discrepancies in the measurements of the main branches of the pulmonary arteries, both in the group of patients and in the group of models, which indicates a good prospect for the routine use of the MSCT method in the aspect of studying the anatomy of the pulmonary arteries in patients before the Fontan operation.
